The Foundation's nationwide database is a tremendous resource for researchers, corporations, and health care organizations that need to understand and pinpoint disparities at the zip code level in order to target interventions to eliminate the prevalence of chronic illness among specific populations. Two initiatives, that illustrate the use of the data, the Congressional Kidney Atlas and the Diabetes Atlas, are featured on this web site.
